Currently, we are accepting most Blue Cross Blue Shield plans, including EPS (Expanded Psychiatric Services) plans. Insurance acceptance is subject to change and we recommend that you contact the office to have your insurance coverage verified. The cost of your appointment will vary depending on the coverage your insurance plan provides. We will verify your benefits with your insurance plan, but please be aware that any quote we receive from the insurance is not a guarantee of benefits.
Other office fees are as follows:
- Missed appointments or cancellations with less than 24-hour notice – $50
- Returned check fee – $25 in addition to the amount of the check (the office prefers to take payments via credit card or cash if possible)
- FMLA or Disability Paperwork – $25 and up (this charge depends on the depth of the paperwork and the time it takes the physician to complete the forms)
- Medical Letters – $25

What do I do in the event of an emergency if the office is closed?
Do not wait to contact us, you should contact emergency services immediately – even when the office is open during normal business hours. If you are having any thoughts of harming yourself or others, or if you find yourself in an emergency, please dial 911 right away or visit the nearest emergency room (ER) for immediate assistance. You can also contact Crisis Services of North Alabama Hotline at 256-716-1000, or you can text or call 988 to reach the Suicide & Crisis Lifeline. These hotlines are available 24 hours a day, 7 days a week.
Why do I have to fill out an application to become a new patient?
We try our best to assist as many patients as possible with quality care and mental health treatment. We require the application process for two reasons: First, we want to ensure that our office is a good match for your care. In some instances, the physician may feel that we are not best suited to provide you with the treatment you need and we may recommend finding a more specialized provider for your care. Second, we want to ensure that we can get you scheduled promptly. There are instances where the doctor’s schedule may become full, and we don’t want to have you waiting too long for an appointment. In these events, we recommend that you contact your insurance for assistance in finding a provider that may have more availability in their schedules.
Can I get a copy of my medical records?
We do not advise patients to carry copies of their mental health records due to the sensitivity of the information within the documents. However, if it is necessary for a patient to obtain a copy of their record, we do have a process for requesting those files. Please notify our office of your intent to obtain a copy of your records. We will then forward this request to the doctor so that she can review your files to ensure they are complete and safe to release. Once we have received the approval from the doctor, we will contact you to pick up the records. There are fees associated with getting a copy of your records: a $50 fee for the time the doctor takes to review the records, and a copy fee of $1 per page. We try to complete these requests as quickly as possible, but it may take 10-14 days to complete your records for pickup. Please be aware that there is no charge for forwarding your records to another physician.
